Printable ostomy output log for ileostomy (free PDF)

By the All Things AI team Updated

If you have an ileostomy, your stoma nurse may ask you to measure and write down what comes out, especially in the first weeks or if output runs high. A clear chart of output, drinks and symptoms helps your care team spot dehydration early and adjust diet or medicines. This sheet has one line per time you empty the pouch and a running 24-hour total.

Not medical advice. This sheet is a way to keep a record for your care team. Follow the instructions your stoma nurse or doctor gave you, and contact them (or emergency services) if you feel unwell.

What stoma nurses and doctors ask for

Thresholds differ between hospitals (1,000, 1,200 and 1,500 mL a day are all used). Use the figure your own team gave you. If you have signs of dehydration, contact your stoma nurse or doctor; in an emergency, call your local emergency number.

Filling it in

  1. Measure with a jug you keep only for this. If you can't measure every time, fill an empty pouch with water once so you know what half-full looks like.
  2. Write drinks in mL too, and note any rehydration solution separately.
  3. Add up each 24 hours at the same time of day so the totals compare.
  4. Bring the sheet to appointments, or photograph it and send it if your team asks.
