Printable pesticide spray record (free PDF)

By the All Things AI team Updated

A spray record answers the questions a state inspector, buyer or worker will ask: what was applied, where, when, how much, and when it is safe to go back in. EPA's Worker Protection Standard lists the application details farms must display and keep, and many states have their own record rules. This sheet records one application per line.

What inspectors and the Worker Protection Standard expect

How long to keep it: WPS: employers keep application and hazard information for 2 years from the end of the REI. Federal statute: 2 years for restricted-use pesticide records where no state rule applies. The former 7 CFR 110.3 also set 2 years. 40 CFR 170.311(b)(6); 7 U.S.C. 136i-1

Not affiliated with EPA, USDA or any state agency. Federal USDA recordkeeping for restricted-use pesticides (7 CFR part 110) was rescinded in July 2025; state rules, the Worker Protection Standard and product labels still apply.

Filling it in

  1. Copy the product name and EPA Registration Number from the label, not from memory.
  2. Record the start and end time, not only the date; the restricted-entry interval runs from the end of the application.
  3. Check your state's pesticide rules too; many require more fields or longer retention than the federal minimum.
  4. Keep the sheet with your labels and safety data sheets so the information display is easy to update.
