What My Supporters does
My Supporters is the free, account-free, fully on-device way to build a legal Supported Decision-Making (SDM) Agreement — the document that lets an adult with an intellectual or developmental disability keep their legal rights and name trusted people to help with specific decisions, instead of being placed under guardianship.
At age 18, IDD-transition families face a fork: petition a court for guardianship, which strips legal rights, or set up Supported Decision-Making, where the person keeps their rights and names supporters. As of 2026, Kansas and Idaho enacted Uniform Guardianship Act provisions requiring probate courts to consider SDM as a less-restrictive alternative BEFORE appointing a guardian — joining Utah, Hawaii, and New Mexico, which all passed SDM statutes in 2025. Twenty-four-plus states plus DC now have SDM laws. The newest ones force courts to actually look for a signed agreement, so families must now produce the document, not just understand the concept.
The catch: today the only ways to make one are to fill out a legal-aid PDF or Word form by hand, or type your information into a state disability-agency WEB form that submits it to a server. That document holds the most sensitive data a person has — a disability diagnosis, protected health info, financial records, and the identities of the people they trust most. Advocates specifically warn this cohort against putting that into cloud or account-based tools, where a removed or contested supporter leaves a server-side trail.
My Supporters runs the whole agreement on the device, with no account and nothing uploaded:
— Supporter builder. Add one or more supporters with name, address, phone, and email. — Statutory decision-area checklist. Initial exactly which life areas each supporter may help with — food/clothing/shelter, physical and emotional health, financial affairs, public benefits, finding work, residential services, school, self-advocacy, and other — mirroring the real Texas, Illinois, and ACLU model forms. — Release toggles. Turn HIPAA, FERPA, and state-confidentiality releases on or off, each with a plain-language explanation of what it means. — Dates and signing. Set effective and end dates, and follow a witnesses-vs-notary checklist so the agreement is executed correctly. — Generate the agreement. Produce a clean, print-and-sign Supported Decision-Making Agreement from your entries, rendered entirely on-device. — Wallet card. Print a quick-reference card showing each supporter and the exact areas they support.
There is no SDM-agreement app on the App Store — every search returns generic pros-and-cons decision pickers. The only digital tools are web forms that send your diagnosis, HIPAA, and financial data to a server. Apple's Notes, Pages, and Reminders cannot model the statutory checkboxes, the release toggles, or the witness/notary block. My Supporters is free, no account, on-device — for IDD adults and the families who help them.
